Job Summary:
The Care Manager will provide comprehensive risk management services for youth and families within the Yunion’s program models, primarily the Cave of Adullam Transformational Training Academy, while also supporting other organizational initiatives. This role requires expertise in youth development, psychology, social work, and related fields. The Care Manager will manage a caseload of students, perform intakes and assessments, provide individual and family counseling, and coordinate services to meet the diverse needs of our clients.

 

Key Responsibilities:

  • Case Management: Manage a primary caseload within the Cave of Adullam Transformational Training Academy, while supporting other youth and families across the Yunion's program models.
  • Counseling and Crisis Intervention: Provide individual and family counseling, perform crisis intervention, and offer support for at-risk youth and their families.
  • Intake and Assessment: Conduct thorough intakes, assessments, and treatment plans to determine the needs of youth and families, creating tailored care plans.
  • Coordination of Services: Collaborate with school administrators, teachers, and parents to coordinate care and support for students, addressing needs such as housing, transportation, healthcare, and mental health services.
  • Community Collaboration: Build and maintain relationships with community partners to enhance the support network for youth and families.
  • Documentation and Reporting: Maintain accurate and confidential records of all client interactions, progress, and outcomes.

 

Minimum Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Counseling, or a related field (Master’s degree preferred).
  • Relevant certifications or training in counseling or crisis intervention.
  • Minimum of 2 years of experience in youth development, social work, juvenile justice, or related fields.
  • Experience working with emotionally disturbed or high-risk youth.
  • Strong coordination and organizational skills.
  • Experience in community-based settings and building collaborative partnerships.
  • Excellent communication and interpersonal skills.

 

Preferred Qualifications:

  • A master’s of social work, public health, or psychology degree from an approved accredited University or College with a training focus on counseling.

      • Ability to perform a wide variety of tasks with accuracy and speed under the pressure of time sensitive deadlines.
      • Ability to be flexible and adapt to organizational changes and needs.
      • Ability to work independently
      • Ability to collaborate and to work as part of a team

 Pay: $40,000 - $50,000 (commensurate with experience)
